for all families with students registered in school
and new student registration.
Reminder: All students who will participate in a St. Clare School athletic program during the 2021-2022 school year must have a yearly physical exam before beginning practices or games. The Physical Exam required by the State of Illinois for grades kindergarten and six and for new students moving from outside Illinois will suffice for the sports physical provided the physician has checked the appropriate boxes on the form.
